鶹Ժ, faculty and staffwho are interested in receiving the COVID-19 vaccine through CU Boulder can now sign up using the CU Boulder .
Please note, you must complete the form to add yourself to the campus vaccine eligibility list.
Information gathered will be used for vaccine distribution as the state proceeds in moving to new phases of eligibility. The form requires signing in with your IdentiKey username and password.

By completing the sign-up form, you will be placed on an eligibility list for receiving the COVID-19 vaccine through CU Boulder and will be notified when it’s time to schedule an appointment.

You must complete the vaccine sign-up form to be notified via email when you are eligible to schedule an appointment to receive the vaccine through CU Boulder.

You will not be able to get a COVID-19 vaccine through CU Boulder until you complete the form and are added to the vaccine list.

You may still sign up to receive the COVID-19 vaccine elsewhere. You are encouraged to get the vaccine at your first opportunity.
When to complete the form
鶹Ժ, faculty and staffwill need to login using their CU IdentiKey username and password to complete the form. By completing the form you can:

Add or update your information for CU Boulder’s vaccine eligibility list.

Schedule a second dose if you’ve received your first dose elsewhere (students only).

Remove yourself from CU Boulder’s vaccine eligibility list (no action needed if you haven’t already signed up).
COVID-19 vaccine distribution at CU Boulder
At this time, the Colorado Department of Public Health and Environment notifies the campus on a weekly basis if it will receive any vaccine shipments. The state is solely responsible for making these determinations. Based on the amount the campus receives from the state, the campus will notify a corresponding number of eligible campus affiliates with information about making an appointment.
If you prefer to not receive the COVID-19 vaccine through CU Boulder, no action is needed. If you sign up and later wish to remove yourself from the eligibility list, you can complete the sign-up form again and select the option “OPT-OUT.” You can opt back in at any time by completing the form again and adding yourself to the CU Boulder vaccine list.
State updates to phases of eligibility

: Individuals in phases 1A, 1B.1 and 1B.2 are currently eligible in Colorado. Phase 1B.3 is estimated to begin March 5.

Late March (estimate): Phase 1B.4 begins. Student-facing employees in higher education and campus affiliates age 50 and over will be eligible in phase 1B.4, in addition to frontline essential workers in a variety of categories and people ages 16–49 with at least one high-risk condition.

Completion of the form is based on self-disclosure and includes a self-attestation where you certify that the information you submit is true and correct.
